Real, and very localized. Not a citywide problem — a three-locations problem: Tram 22, Livraria Lello queue, Mercado do Bolhão peak hours. The adaptation is small: cross-body bag with the zip in front, phone in pocket not in hand, in those specific spots. Outside those three places, Porto is one of the most relaxed European cities for personal belongings. Phone on the café table is fine.
